Immunizations
To ensure the health and safety of our campus, immunization against communicable diseases is extremely important. The following vaccinations are required of all first-time freshmen, and all transfer students:
- MMR (Measles, Mumps, Rubella), two doses
- Meningococcal Vaccine (if under age 21), within five years
- Polio, three doses
- Tetanus (TDaP), most recent within ten years
- Hepatitis B, three doses
If you are enrolled in a healthcare major, you may have additional immunization and reporting requirements. Contact your department with questions about additional requirements.
Additionally, the following immunizations are recommended prior to enrollment at WVU:
- Varicella vaccine (chicken pox)
- Tuberculosis screening
- Hepatitis A vaccine or combined Hepatitis A&B vaccine
- Annual Influenza vaccines
- Pneumovax, if indicated
- HPV series – Gardasil (for females and males)
- COVID-19
How to Submit Your Immunization Information
Immunization information is submitted entirely online at WVU and WVU Tech. There are two options for submission of your immunization information:
If you will be opting out of the WVU-sponsored health insurance plan by completing
the student health insurance waiver online:
Immunization confirmation questions are integrated into the
student health insurance waiver. Completing the student insurance waiver
and confirming that you received all required immunizations satisfies the immunization
requirement. Immunization dates and a physician’s signature are not required.
If you will not be completing the student health insurance waiver:
Please complete the Immunization Questionnaire below prior to arriving on campus.
Please note that you will be required to log in to the Immunization Questionnaire
using your WVU Login and password. Your Login will not be active until
you have registered for classes. Claim your Login if you have not already. If you do
not complete the student health insurance waiver and are eligible, you will be automatically enrolled in the WVU-sponsored health insurance
plan.
