Release of Information

Authorization for Release of Employee Benefits Information allows employees to designate who can discuss their benefits related information with the Employee Benefits Staff. Employees can designate multiple people to discuss all their benefits related information or only specific areas.

If an employee does not submit an Authorization for Release of Information Form, Benefits Strategy nor WVU Shared Services cannot discuss any information with anyone other than the employee.

Although it may be updated at any time, Benefits Strategy encourages employees to update this form after a Qualifying Life Event.
