Coverage Options: Two coverage options are available.
- Employee - employee is the only one covered.
- Employee + Family - employee plus family members you support are covered (there is no age limit for children).

Premiums: Premiums are billed directly from the carrier to the enrolled participant and are not paid by payroll deduction.

- Employee + Family - $13.95/month
